**Job Summary:**
Bay-CSS is seeking a compassionate and dedicated individual to fill the position of CDS/Meaningful Day with Transportation in Lexington Park. The primary responsibility of this role is to support individuals with disabilities in participating in meaningful day activities and ensuring safe transportation to and from these activities.

**Qualifications:**
- High school diploma or equivalent required
- Valid driver's license and a clean driving record
- Experience working with individuals with disabilities preferred
- Strong communication and interpersonal skills
- Ability to handle physical demands of the job, including assisting individuals with mobility needs

**Responsibilities:**
- Assist individuals in engaging in meaningful day activities, such as vocational training, recreational outings, and community events
- Provide supervision and support to ensure the safety and well-being of individuals during activities
- Drive individuals to and from activities in a safe and timely manner
- Maintain documentation related to daily activities, transportation logs, and individual progress
- Collaborate with other team members to create and implement individualized plans for each participant
