- Job Title: Home Health LPN
- Pay: $34.00- 37.00 an hour paid weekly + mileage reimbursement
- Location: Kentucky – Counties Covered: Daviess, McLean, Ohio, Webster, Henderson
- Shift: Monday–Friday, 8:00 AM – 5:00 PM (No calls, no weekends)
- Benefits: available to full-time employees after 90 days of employment
Position Overview:
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality home health care for patients in Kentucky. The LPN will follow the client’s plan of care, monitor vital signs, administer medications, and collaborate with other healthcare professionals to ensure patients receive comprehensive and compassionate care.
Key Responsibilities:
- Provide nursing care according to the client’s plan of care while maintaining dignity and independence.
- Monitor vital signs and report abnormal findings.
- Administer medications, feedings, oxygen, and ostomy care as needed.
- Assist with routines established by PT, OT, and Speech Therapists.
- Maintain infection control procedures and report possible communicable diseases.
- Document nursing actions, patient progress, and outcomes accurately.
- Order supplies, medications, and feedings as needed.
- Ensure all patient needs are met in accordance with physician instructions.
- Monitor and report incidents, completing incident reports timely and accurately.
- Maintain medical equipment and report faulty equipment for repair.
Qualifications:
- Licensed Practical Nurse (LPN) – Active Kentucky license required
- Minimum 1-year home health experience preferred
- Skilled in wound care and general LPN nursing tasks
- Experience with Homecare Homebase (HCHB)
