Job Description
HealthCare Support is actively seeking a RN Long Term Care Manager - Hybrid to fill an opening New Castle County, Delaware.
Daily Responsibilities for RN Long Term Care Manager - Hybrid:
- Position purpose: Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.
- Evaluates the needs of the member, the resources available, and recommends and/or facilitates the plan for the best outcome
- Assists with developing ongoing long-term care plans/service plans and works to identify providers, specialist, and/or community resources needed for long-term care
- M-F 8AM-5PM EST
Required Qualifications for RN Long Term Care Manager - Hybrid:
- Bachelor’s degree
- Must have active and valid DE RN license
- 2+ years of case management experience and completing field visits for long term care Medicare/Medicaid, Home Health, or older/disabled adults
- Experience with medication reconciliation
Preferred Qualifications for RN Long Term Care Manager - Hybrid:
- Exp with geriatric population - highly preferred
- Exp working with TruCare
- Knowledge of healthcare and managed care preferred

Pay Details: $36- $40/ Hour
