Job Summary
Nurse Case Managers ensure the individuals we support receive excellent health services. You'll monitor complex medical needs, collaborate with interdisciplinary teams, mentor unlicensed staff, and serve as a trusted liaison between families, guardians, and healthcare professionals.
Key Responsibilities
- Assess and monitor the health of assigned individuals
- Order and track medications, equipment, and medical supplies
- Develop, implement, and revise individualized healthcare plans
- Ensure documentation is current and in compliance with physician orders, and DHS licensing standards
- Present health summaries at annual IDT meetings and write quarterly/annual evaluations
- Provide training and oversight for delegated nursing tasks
- Respond to medical emergencies and serve in on-call phone rotation
- Maintain strong, respectful communication with families and care teams
Required Qualifications
- Current LPN or RN license in the State of Minnesota
- Current CPR and First Aid certification
- One year of experience working with individuals with developmental disabilities and complex medical needs
- Valid Minnesota driver’s license and acceptable driving record
- Must pass a MN Department of Human Services background check
- Ability to lift up to 60 lbs and meet the physical demands of the job
Preferred Skills
- Knowledge of DHS and ICF/DD regulations
- Familiarity with electronic medical records (Therap preferred)
- Excellent communication and documentation skills
- Strong problem-solving and time management abilities
- Passion for advocating for individuals with disabilities
