Catholic Charities of St. Paul and Minneapolis is looking for caring, compassionate individuals to ensures older adults and individuals with disabilities live and thrive in the home of their choosing! WAGE RANGE: $56,000 - $64,000 annually
LOCATION: This role is primarily work from home but requires 5-15 client visits per month.

SUMMARY:
The Homeless Elders Program provides in-person outreach and housing-focused case management to older adults experiencing homelessness with the goal of assisting them with obtaining safe, stable housing and appropriate support. This is an exciting and unique position, providing person-centered care to some of the most vulnerable in our community. Staff each have a revolving caseload of approximately 20 older adults in shelter or highly unstable housing, working closely with them to work through barriers to housing as well as other goals as determined by a person-centered care plan. Staff may also assist with conducting trainings to internal and external providers on working with homeless older adults.
ESSENTIAL FUNCTIONS:
- Provide comprehensive case management services including:
- Intake, interview and assessment
- Goal setting
- Information and referral services
- Provide assessment services and short-term counseling, skill building techniques, and aftercare.
- Provide client services in a variety of outreach settings both at agency sites and in the community.
- Responsible for documentation, reviewing and ensuring the relevance, accuracy, and timeliness of data in client files.
- Make appropriate referrals and provide comprehensive decision-making counseling to help ensure client’s needs are being met.
- Enroll clients in Housing Stabilization Services for billing purposes as appropriate
MINIMUM QUALIFICATIONS/EXPERTISE:
- Current Social Work licensure approved by the State of Minnesota required (e.g. LSW, LGSW).
- Bachelor’s or master’s degree in social work, Psychology, Sociology, Human Services, or related field preferred.
- Or a bachelor’s degree with a major in any field and at least one (1) year of experience as a social worker/case manager/care coordinator in a public or private social services agency.
- Pass DHS background check and meet their requirements for the provision of case management by meeting the social work standards under the MN Merit System.
- Additional training, education, or licensure may be required per program need.
- Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
- Must have a valid driver’s license and proof of current insurance coverage
