Job Title: Care Manager – Emergency Department

Location: Bronx, NY

Duration: 13-Week Contract

Schedule: Day Shift – Monday through Friday

Hours: 9:00 AM – 5:00 PM

Guaranteed Hours: 35 Hours Weekly

Compensation:

  • Pay Rate: $56–$71/hour (Blended Rate)

Position Overview

We are seeking an experienced Registered Nurse Care Manager to support care coordination activities within a busy Emergency Department setting. The Care Manager will be responsible for utilization review, discharge planning, care coordination, and facilitating safe transitions of care. The ideal candidate will possess strong clinical judgment, case management expertise, and the ability to collaborate effectively with interdisciplinary healthcare teams, patients, and families.

Responsibilities

  • Coordinate patient care and facilitate safe, efficient transitions across the continuum of care.
  • Conduct utilization reviews to ensure appropriate level of care and resource utilization.
  • Develop and implement discharge plans that address patient needs and support continuity of care.
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to optimize patient outcomes.
  • Assess patient needs and identify barriers to discharge and care progression.
  • Communicate with patients and families regarding treatment plans, discharge options, and available resources.
  • Facilitate referrals to community resources, rehabilitation services, home care, and other post-acute care providers.
  • Maintain accurate and timely documentation within electronic health record systems.
  • Monitor patient progress and adjust care plans as necessary.
  • Participate in interdisciplinary care planning meetings and case reviews.
  • Ensure compliance with regulatory requirements, organizational policies, and quality standards.
  • Utilize independent clinical judgment to support patient-centered care decisions.

Required QualificationsEducation

  • Associate Degree in Nursing (ASN/ADN) from an accredited nursing program required.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Master’s Degree in Nursing or related healthcare field preferred.

Licensure & Certifications

  • Active Registered Nurse (RN) License in the State of New York required.
  • Current Basic Life Support (BLS) Certification required.
  • PRI and Screen Certification required within three months of employment.
  • License verification through the applicable state licensing authority required.

Experience

  • Minimum five (5) years of clinical experience as a Registered Nurse required.
  • Experience in Care Coordination required.
  • Experience performing Utilization Review required.
  • Experience with Discharge Planning required.
  • Emergency Department, Case Management, or Care Management experience preferred.

Skills & Competencies

  • Strong knowledge of care management and case coordination principles.
  • Experience with utilization management and discharge planning processes.
  • Excellent clinical assessment and critical-thinking skills.
  • Strong communication, interpersonal, and patient advocacy abilities.
  • Ability to collaborate effectively within interdisciplinary healthcare teams.
  • Proficiency with electronic medical records and healthcare documentation systems.
  • Strong organizational, problem-solving, and time-management skills.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • High level of professionalism, accountability, and attention to detail.
