About Us

Bridging Gaps Through Positive Solutions (BGTPS) is a peron-centered mental health organization dedicated to providing a range of therapeutic services, including on-site and community-based services to children, adolescents, and adults in Baltimore City/County.

Position: Behavioral Health Coordinator

Job Types: Full-time, Part-time

Pay: $30.00 - $33.00 per hour

Expected hours: 40 per week

Summary

The Behavioral Health Coordinator (BHC) at Bridging Gaps Through Positive Solutions (BGTPS) is responsible for delivering and coordinating individualized, goal-oriented rehabilitation services to children, adolescents, and adults experiencing behavioral health challenges. Working in home, community, and office-based settings, the BHC conducts comprehensive needs assessments, provides supported living services, skills enhancement, crisis intervention, and supportive counseling to promote independence and integration into community life. This role plays a key part in implementing Individual Rehabilitation Plans (IRPs) and supporting clients on their journey toward wellness and self-sufficiency within the framework of Maryland’s Psychiatric Rehabilitation Program (PRP). This position offers an opportunity to grow professionally within the Social Work, Counseling, Psychology and Human Services field.

Key Responsibilities

  • Provide a minimum of 4–6 direct service sessions per client per month, as planned for by the rehabilitation team (PRP worker and Rehab Specialist) and client
  • Engage clients through home visits, school visits, and community-based sessions
  • Coordinate, develop and implement Individual Rehabilitation Plans (IRPs) for assigned caseload
  • Provide one-on-one and group skill-building sessions focused on life skills, coping strategies, and community functioning
  • Maintain client charts and documentation of all services in accordance with agency policies and COMAR regulations
  • Support clients in accessing community resources such as housing, employment, education, and health services
  • Participate in interdisciplinary team meetings and case conferences to review and update service plans
  • Promote wellness, self-advocacy, and independent living in alignment with treatment goals
  • Help clients resolve problems, identify resources, and make effective use of those resources
  • Assist with client transportation as needed to support rehabilitation goals
  • Perform administrative tasks as required

Minimum Qualifications:

  • Associate or Bachelor’s degree in Social Work, Counseling, Psychology or related field preferred
  • Minimum of 2 year of experience in psychiatric rehabilitation or community-based behavioral health
  • Strong interpersonal, problem-solving, and documentation skills
  • Familiarity with Maryland Medicaid and PRP service standards
  • Ability to manage time effectively and work independently in the field

Preferred Qualifications:

  • Experience working with children, adolescents, and adults in a PRP setting
  • Knowledge of COMAR 10.63, BHA guidelines, and documentation requirements
  • DLA-20 Certified
  • CPR/First Aid certification
  • Must have a valid driver’s license, reliable transportation, and meet background check requirements

Experience:

  • Mental health counseling: 2 years (Required)
  • EMR systems: 2 years (Required)
  • Strong verbal and written communication Skills
  • Assist clients with connecting to vital community resources such as housing, healthcare, education, and employment support.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR) per agency and state requirements.
  • Perform crisis de-escalation and intervention in coordination with clinical supervisors and care teams.
  • Participate in treatment team meetings, trainings, and audits to ensure program quality and compliance.
  • Contribute to a positive, culturally responsive, and equity-focused service environment.
