The HH RN will play a vital role in delivering high-quality nursing care to patients within their homes, focusing on chronic disease management and wound care. This position requires frequent travel to multiple patient homes as part of a collaborative home health care team. The role offers a PRN schedule with potential for full-time employment, supporting individualized patient assessments and care plans to promote optimal health outcomes.

Responsibilities

  • Perform comprehensive patient assessments
  • Develop and implement individualized care plans
  • Provide skilled wound care management
  • Educate patients and caregivers on health management
  • Document all patient interactions and care accurately
  • Coordinate care with interdisciplinary home health team
  • Monitor vital signs and recognize changes in condition
  • Maintain clear and effective communication with patients and team members

Preferred Qualifications

  • Associate's or Bachelor's degree in Nursing
  • Current Registered Nurse (RN) license
  • Home Health experience preferred but not required, willing to train clinicians new to home health!
  • Wellsky/Kinnser experience preferred but not required
