Shift: 0645-1915 Description: Case Manager Start Date: 06/02\nDuration: 12 weeks\nShift: AM/Days\nHours: 40 hours guaranteed | 5 8-hour\nshifts Note: Each week the traveler will be working in Mililani for 4 days and in Kapolei for 1 Occasional home visits are required (approximately 1-3/week) so personal transportation is highly recommended.\nRequired: Active Hawaii RN License \nMinimum 1 year of Primary Case/Care Management experience\nAHA BLS Preferred:Clinic care management experience preferred but hospital case management is acceptable\nHawaii Care/Case Management experience\nCare/Case Management certification\nGuaranteed Hours: Contract Weeks:90
