JOB RESPONSIBILITIES (include, but not limited to the following)
- Provides direct patient care as defined in State’s Name Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
- Maintains verbal and/or written communications among other personnel involved in the care of the patient; maintains clinical records on all patients referred to physical therapy and in consultation with the licensed physical therapist.
- Performs and submits required documentation of patient’s clinical notes, progress reports, summaries and case conferences along with changes in orders. All patient visit notes must be turned in the next scheduled work day, not to exceed forty-eight (48) hours.
- Advances professional growth; participates in training sessions at least 2 times annually; attends short courses, and studies professional journals and texts.
QUALIFICATIONS:
- Graduate of a school of professional nursing and is currently licensed, certified, or registered in the state(s) in which he/she practices.
- Should have good communication skills.
- CPR certification and valid driver’s license.
- Ability to work well with others, function well under pressure, and function independently.
- Must comply with the standards outlined in the Nurse Practice Act. Acts only with the scope of their state license or state certification or registration.
- LVN must keep their qualifications current at all times. Current State License to practice in Texas as a Licensed Vocational/Practical Nurse
Language:
- Bilingual English/Spanish preferred
