Hudson MD Group, LLC (“Practice”) is a New Jersey limited liability company operating as a large multi-specialty medical group practice comprised of more than one hundred twenty primary care and specialist physicians practicing in New Jersey. Practice is dedicated to delivering high-quality, patient-centered care. We recruit over 50 providers a year and are seeking a skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our practice.
Job Summary
We are looking for a compassionate and experienced Nurse Practitioner or Physician Assistant to provide exceptional patient care in a collaborative and innovative environment. The ideal candidate will have experience in [insert specialty, e.g., primary care, urology, men's health, etc.] and be comfortable working in a hybrid concierge model that integrates traditional insurance with value-added services.
Key Responsibilities
- Monitor and manage quality metrics, including HEDIS measures, to improve patient outcomes
- Conduct annual wellness visits and physicals providing comprehensive patient assessments, diagnose, and develop treatment plans
- Provide preventive care, chronic disease management, and health education
- Collaborate with physicians and other healthcare professionals for coordinated care
- Perform minor procedures as needed within the scope of practice
- Must be familiar with electronic health record (EHR) to to be able to document patient encounters accurately.
Qualifications
- Certified Nurse Practitioner (NP) or Physician Assistant (PA) with valid New Jersey licensure
- Minimum of two (2) years of experience primary care.
- Strong clinical and diagnostic skills
- Excellent communication and interpersonal skills
- Ability to work independently while collaborating in a team-based environment
- Fluency in other languages, such as Korean, Chinese, Hindi, Russian and Spanish is a plus
