Position: Choices Care Coordinator

Department/Program: Choices for Care (CHOICES #600) STATUS: Full-time Part-time Exempt Non-exempt

Reports to: Choices for Care Supervisor

Solutions for Care (SFC) serves over 12,000 older adults and those living with disabilities each year in Berwyn, Cicero and North Proviso Township. We provide direct client service through information and assistance, in-home care, advocacy services and transitional assistance in hospitals. General Statement of Duties:

Completes Choices for Care assessments for various programs and services including; homemaker, adult day services, assisted living, and nursing facility placement. Provides planning, problem solving, benefit linkage and referral to local resources. Promote problem solving, family system integrity and client independence. All assessments are conducted at hospitals or nursing homes in the Berwyn, Cicero and North Proviso areas.

Qualifications:

  • Bi-lingual in Spanish/English is preferred.
  • Minimum qualifications include a BA/BS degree in Social Science, Social Work, or related field, or an RN, or a BSN.
  • One year of program experience, defined as assessment, provision, and/or authorization of formal services for the elderly, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree; or be an LPN with one year of program experience which is defined as assessment of and provision of formal services for the elderly and/or authorizing service provision.

Requirements:

  • Strong organizational and time management skills.
  • Adaptability to changing work conditions, that includes working on weekends on a rotating schedule.
  • Strong communication skills and the ability to work with diverse populations.
  • Proficiency with Microsoft Office (Word, Excel, and Outlook) to maintain accurate case notes, reports and record keeping.
  • Comfortable multi-tasking on the computer using different screens.
  • Must have a vehicle, valid driver’s license, and vehicle insurance for local travel to home visits, hospitals, trainings, and meetings.
  • Criminal background check required.

Training:

Must attend and pass the state mandated Illinois Department on Aging training which may require travel to Springfield, Lombard or Chicago, Illinois.

Duties and Responsibilities:

  1. Develop excellent knowledge of programs, benefits and services available to seniors on a local, regional, state and federal level.
  1. Gain and understanding of program policies and procedures.
  2. Meet all funding source training and certification requirements for this position.
  1. Manage assigned clients including accurate and timely documentation of all interviews for each client – including case note entry, data reports and record keeping.
  2. Advocate for the client and assist in problem solving, family system intervention, benefit assistance and support of clients’ rights and choices.
  1. Will complete approximately 100 patient assessments per month.
  2. Maintain written weekly schedule as required by supervisor.

  • Ability to lift up to 20 pounds.
  • Must be able to sit and work at workstation for extended periods of time.
  • Must be able to walk through the hospital throughout the day
    • Must be comfortable conducting visits at hospitals, nursing facility, supportive living and rehabilitation center, etc.
      • Must be able to walk, climb stairs, use elevators, and conduct visits

Schedule:

Full time: 8:30 am to 4:30 pm, Monday through Friday and Rotating weekends.


Salary: $42,500 to $45,000. During the first year, six-month and 12-month performance reviews are conducted. Thereafter, performance reviews will be conducted every 12 months. Salary increases are based on performance.
