Bridging Care is a Care Coordination Organization serving the underserved Medicare & Medicaid population through Washington State’s Health Homes program. Our mission is simple but powerful: to bridge gaps in healthcare and improve lives for those who need it most. We are a growing, mission-driven team, expanding from 22 to 60+ employees in just two years and we’re looking for passionate, organized, and driven Care Coordinators, Medical Assistants, Case Managers, Social Workers, and Nurses to join us!

What You’ll Do
Manage a caseload of Medicare/Medicaid clients - you will begin with 30 cases and be at full case load of 55+ by month 3).
• Provide care coordination through Face-to-Face in-person visits (home visits).
• Create Health Action Plans; implement short term goals to reach members’ long-term goals.
• Meet monthly performance expectations (e.g., 90% of caseload touched/documented).
• Document all interactions in compliance with program and leads’ standards.
Your Training & Ramp-Up (Designed for Success)
• Weeks 1–4: Meet with Leadership, Virtual modules, guided trainings, state program training.
• Week 5: Shadow experienced Care Coordinators in the field.
• Week 6: Mentor shadows you to ensure confidence and readiness.
• Month 2–3: Begin with 30 cases, gradually building to a full caseload of 55+ by the end of month 3.
What We’re Looking For
· Background & Experience: Prior experience in healthcare, field work, case management, or social services is preferred.
· Organization & Accountability: Strong organizational skills with the ability to work toward goals, manage checklists, and meet accountability standards.
· Compassionate Approach: Genuine empathy and commitment to serving underserved populations.
· Technical Proficiency: Comfortable using technology such as telehealth platforms, documentation systems, Microsoft Word, Excel, and Teams.
· AI Readiness: Willingness to work with and learn from AI tools—Bridging Care is a forward-thinking AI-enabled company.
· Education: Bachelor’s degree in healthcare, social work, or a related field is preferred (or equivalent relevant experience).
