ESSENTIAL FUNCTIONS:
- Provide medical assessments and acute care services to the Department of Homeless Services pediatric, adolescent, and adult applicants.
- Provide and document adequate assessment details and relevant patient history.
- Complete medical triage health screening forms via CARES.
- Provide counseling for Nicotine Replacement Therapy “NRT” and Safe Sleeping “SIDS.
- Use appropriate and approved reference material, Up to Date, to provide relevant pediatric and adult patient care at PATH.
Requirements:
New York State Nurse Practitioner license; minimum two years’ clinical experience, familiarity with word processing and other computer applications; ability to speak Spanish preferable.
