Key Responsibilities

  • As a Primary Care Physician (PCP) and leader of the care team, you will provide comprehensive primary healthcare to a dedicated patient panel while ensuring exceptional, value-based care to the community.
  • Deliver patient-centered, comprehensive primary care services aligned with a value-based care model
  • Actively participate in quality improvement programs and contribute to practice performance metrics
  • Maintain compliance with privacy, regulatory, and quality standards, ensuring adherence to HealthyU Family Medicine policies, procedures, and protocols
  • Implement and support innovative healthcare technologies, services, and programs to enhance community health and wellness
  • Collaborate with clinical and administrative teams to ensure high-quality patient care
  • Perform other duties as assigned

Requirements:

  • Completion of an accredited Medical Program in Family Medicine or Internal Medicine
  • Valid state-issued medical license (must be active by the first date of employment)
  • Current BLS (Basic Life Support) certification
  • Active DEA Registration
  • Ability to provide two professional references
  • Two or more years of clinical experience in primary care
  • Experience providing longitudinal patient care (continuity of care)
  • Familiarity with managed healthcare systems, including quality assurance, quality improvement, risk management, population health, and disease management programs
  • Strong ability to work collaboratively within a multidisciplinary team
  • Excellent communication skills with both patients and colleagues
  • Self-motivated and results-driven approach to patient care
