Position: Licensed Practice Nurse (LPN) - Hospice

Supervisor: Hospice Manager



Job Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care to Home Health and Hospice patients, families, and caregivers under the direction of a Registered Nurse. This position ensures that patient care is delivered in accordance with the plan of care, Medicare Conditions of Participation, Pennsylvania licensure regulations, and agency policies. The LPN plays a critical role in supporting patients in the home environment, promoting comfort, healing, and independence.

Key Responsibilities

Patient Care 

  • Under the direction of an RN, delivers safe, high-quality nursing care as outlined in the patient’s plan of care.
  • Administer medications and treatments in accordance with the PA Nurse Practice Act.
  • Monitor and report patient responses to medications and treatments.
  • Assess and educate patients and caregivers on disease management, medications, and self-care techniques.
  • Assist patients with activities of daily living, promoting independence and comfort.
  • Observe for changes in condition and report to RN Case Manager or Clinical Manager.
  • Maintain timely and accurate documentation of all care provided.
  • Participate in coordination of care with the interdisciplinary team to ensure continuity.
  • Provide visits to Hospice patients as needed.

Team Collaboration & Communication

  • Communicate effectively and respectfully with patients, caregivers, and all members of the care team.
  • Serve as a mentor during orientation or field training as requested. 
  • Promote a supportive and respectful work environment aligned with agency values. 
  • Attend staff meetings, case conferences, and required in-services. 

Regulatory & Professional Standards 

  • Maintain compliance with all local, state, and federal regulations. 
  • Comply with Medicare Home Health Conditions of Participation and agency procedures. 
  • Maintain confidentiality in all patient and agency-related matters. 
  • Uphold professional and ethical conduct in all responsibilities. 
  • Participate in agency quality improvement initiatives and documentation reviews. 

Resource Management 

  • Coordinate visits efficiently and manage caseloads cost-effectively. 
  • Maintain clinical records according to agency standards. 
  • Participate in weekend/evening rotation as required. 

Knowledge, Skills, and Abilities 

  • Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records
  • Excellent in verbal and written communication, organization, and time management skills 
  • Proficiency with Microsoft Office Suite and EHR systems
  • Ability to work independently and collaboratively in a fast-paced environment 
  • Compassionate, patient‑centered approach with sound clinical judgment 
  • Excellent interpersonal, negotiation, and conflict resolution skills.

  • Requires travel within the agency’s service area to provide care in patient homes and community settings. Exposure to varying home environments and conditions.
  • Ability to lift up to 50 pounds occasionally
  • Adequate visual/hearing ability for patient care and communication

Education and Other Requirements

  • Graduate of an accredited practical nursing program and currently Licensed Practical Nurse (LPN) in Pennsylvania
  • One year of clinical experience preferred (Home Health, Med/Surg, or critical care)
  • CPR certification required
  • A valid driver’s license, reliable transportation, proof of auto insurance with acceptable driving history is required.

Monday - Friday 8:00 - 4:30. Occasional weekends may be needed.
