Healing Home Healthcare is seeking a skilled Home Health Physical Therapist to join our team. We provide high-quality care to patients in their homes and offer a rewarding, dynamic work environment.
Position: Home Health Physical Therapist
Employment Type: Full-time/Part-time
Coverage Area: Northen Virginia; Culpepper, Marshall, The Plains, Hume, Linden, Manassas, Woodbridge, Gainsville
Responsibilities:
Conduct comprehensive evaluations of patients' physical and functional abilities
Develop personalized treatment plans to enhance mobility, relieve pain, and improve overall physical well-being
Provide hands-on therapy and exercises to patients in their home settings
Educate patients and their families on therapeutic exercises and techniques
Collaborate with interdisciplinary healthcare professionals to coordinate patient care
Document and maintain accurate records of patient progress and treatment outcomes
Qualifications:
Valid license as a Physical Therapist in Virginia.
Experience in home health or a related field is preferred but not mandatory
Strong clinical skills and knowledge of evidence-based physical therapy practices
Excellent communication and interpersonal skills to effectively interact with patients, families, and other healthcare team members
Self-motivated, compassionate, and dedicated to providing exceptional patient care
Ability to work independently and manage time efficiently
Valid driver's license and reliable transportation
