Activities / Memory Care Director

  • Start Date: ASAP, within the next few weeks
  • Salary: starting salary based on experience & credentials
  • Shift/Schedule: This is a full time day shift position with Manager on Duty (MOD) weekend rotations

The Terrace is a classic Skilled Nursing Facility that has 115 beds, with a heavy emphasis on long term resident care and a small, but growing, short-term rehab unit. This close-knit team is welcoming and always willing to jump in to help out when you need it. What makes us unique:

  • 4-Star Quality measure – this means we provide excellent clinical care!
  • 30% of our residents are late stage memory care patients and we are one of the only nursing homes in the area with dementia programming.
  • We have a busy Activities Department that keeps our residents engaged with low level sensory rooms, cognitive therapeutic games, regular entertainment and cooking classes.

Job Description

What you will do in this role: When you are a patient, games and entertainment can be the highlight of your day. As an Activity / Activities Director you will design a curriculum to keep our short-term residents interested & engaged in their recovery and rehab. For our long-term residents, you will create life enrichment events, often focused on cognitive and memory retention. This could involve leading exercise groups, managing a team of Aides, and above all, making this a stimulating environment for all.

Qualifications

Sometimes this position is referred to as a center's Alzheimer's Unit Manager or Memory Care Director.

Here's what we need from you:

  • Ideally, 1-2+ years prior experience in a skilled nursing facility as a Memory Care Manager (or similar title) or Activities Director
  • Dementia Care training and experience is required
  • Certified Activity Director (the 36-hour certification course for the state of IL)
  • Electronic medical records experience (Point Click Care or PCC) is ideal

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
