Job Description

We are seeking a compassionate and experienced Triage RN to join our Hospice team. This role is essential to delivering timely, patient-centered support by providing telephone-based symptom assessments, coordinating care, and serving as a clinical resource for field staff. You'll collaborate closely with patients, families, clinicians, and facility partners to ensure exceptional hospice care and symptom management.

Location: Office-Based in Ridgefield Connecticut; Hybrid/Remote After Training

Schedule: Monday–Friday, 8:00 AM–5:00 PM (1-hour lunch, 37.5 hours/week)

Key Responsibilities:

  • Serve as the central point of contact for triaging inbound calls from hospice patients, families, and facility staff

  • Provide clinical assessments over the phone, determining urgency and next steps in collaboration with the care team

  • Coordinate home visits, urgent interventions, and medication changes based on clinical needs

  • Maintain timely and accurate documentation in the EMR (HomeCare HomeBase)

  • Receive and deliver shift change reports, ensuring continuity of care across the team

  • Collaborate with hospice team coordinators, field nurses, and physicians to manage visit scheduling and patient follow-up

  • Perform chart audits and assist with clinical workflow tasks, such as pushing new admissions (SOC) to calendars, entering physician orders, and monitoring compliance timelines

  • Participate in periodic in-person team meetings and provide in-office coverage as requested

  • Contribute to continuous improvement by identifying workflow challenges and supporting quality initiatives

What We're Looking For:

  • Licensed Registered Nurse in the State of Connecticut

  • Hospice nursing experience required; strong knowledge of hospice philosophy and end-of-life symptom management

  • BSN and/or CHPN certification preferred

  • Strong telephone triage and clinical assessment skills

  • Confident decision-maker with the ability to remain calm and compassionate during emotionally charged or time-sensitive situations

  • Proficient with clinical documentation and EMR systems (HomeCare HomeBase experience a plus)

  • Understanding of Medicare Conditions of Participation, HIPAA, and CHAP Standards

  • Excellent interpersonal, organizational, and communication skills
