Job Title: Adult / Gerontology Nurse Practitioner
Location: Manhattan, NY
Shift: Full & Part-time available
Responsibilities:
· Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
· Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident
· Order & interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning
· Participate in care initiation for our chronic care management services.
Requirements:
· Master's degree in Nursing and graduate of an accredited Nurse Practitioner program
· Unrestricted New York state license as a ANP / AGNP.
· Unrestricted DEA Certification
· Experience working with geriatric population and/or long-term care.
· Eligible to provide reimbursable services and maintain provided status with Medicare and Medicaid
· Minimum 3 years of experience; preferably in long-term care
· Experience with eClinicalWorks (eCW) electronic health records
