Embassy of Heartside is seeking a compassionate full-time Physical Therapist (PT) to join our in-house rehabilitation team in State College, PA.
This is an excellent opportunity for a PT who is passionate about helping residents improve mobility, strength, balance, and overall independence. As part of our collaborative rehabilitation team, you will provide personalized, resident-centered therapy and develop individualized treatment plans that support meaningful progress, improved function, and a higher quality of life. Key Responsibilities
• Evaluate residents’ mobility, strength, balance, and functional needs
• Develop and implement individualized physical therapy treatment plans
• Provide skilled therapy to reduce pain, restore function, and promote healing
• Monitor resident progress and adjust treatment plans as needed
• Educate residents, families, and caregivers
• Collaborate with the interdisciplinary care team
• Maintain accurate, timely, and compliant documentation
• Ensure services meet all professional, legal, and regulatory standards
Qualifications
• Active Physical Therapy license in PA, in good standing
• Skilled nursing or long-term care experience preferred
• Strong clinical, communication, and organizational skills
• Knowledge of the Net Health Electronic Documentation System preferred
