Location: New York City (Brooklyn, Queens, Manhattan)

Start Date: July 1, 2026

Compensation: $22 - $23 per hour, grant-funded, with the possibility of becoming full-time staff after the grant concludes.


Position Overview:

As part of New York State’s 1115 Medicaid Waiver initiative, the HRSN Screener/Navigator plays a pivotal role in addressing social determinants of health. This position involves conducting standardized screenings to identify unmet health-related social needs among Medicaid members and facilitating connections to appropriate services. The goal is to enhance health equity and improve outcomes by integrating social care into the healthcare delivery system.


Key Responsibilities:

· Administer the Accountable Health Communities (AHC) screening tool to assess members' health-related social needs.

· Evaluate eligibility for enhanced HRSN services based on screening results and established criteria.

· Navigate members to appropriate services, including housing support, nutrition assistance, transportation, and care management.

· Utilize closed-loop referral systems to ensure successful service connections and follow-ups.

· Collaborate with healthcare providers, community-based organizations (CBOs), and other stakeholders to coordinate care.

· Maintain accurate and timely documentation of all interactions, assessments, and referrals in compliance with program standards.

· Participate in ongoing training and professional development opportunities related to HRSN and social care navigation

Requirements:

Education:

· An associate’s or Bachelor’s degree in health, social services, or a related field is preferred.


Experience:

· New graduates are welcome.


Skills:

· Understanding of health equity and social determinants of health.

· Excellent communication and interpersonal skills.

· Proficiency in navigating software platforms.

· Ability to work independently and as part of a multidisciplinary team.

· Bilingual abilities (e.g., Chinese or Spanish) are highly desirable.

· Knowledge of community resources and social service programs in New York City.
