Job DescriptionAbout This RoleThe Enrollment Specialist plays a key role in connecting older adults and their families to myPlace Health’s PACE (Program of All-Inclusive Care for the Elderly). You will help bring our mission to underserved communities across Los Angeles County, ensuring that every new participant experiences a smooth, supportive, and compassionate enrollment journey.
As the link between the Outreach Team and the Interdisciplinary Team (IDT), you’ll help coordinate care, streamline communication, and ensure every detail of the enrollment process is handled with care. Your work directly supports our vision of helping frail older adults live independently and with dignity in the community they love.What Does Success Look Like:

  • Champion Seamless Enrollment: Work hand-in-hand with our Outreach and Enrollment Team to guide prospective participants through a smooth and supportive enrollment experience.
  • Partner Across Care Teams: Collaborate closely with members of our Interdisciplinary Team, including nurses, providers, and care coordinators to ensure every new participant transitions into our program with confidence and care.
  • Keep Every Detail on Track: Maintain accurate documentation and record all enrollments in our CRM system, helping us stay organized, compliant, and participant-focused.
  • Uphold the Highest Standards: Adhere strictly to DHCS and CMS regulations and policies to ensure ethical marketing practices and full compliance for the PACE program.
  • Drive Enrollment Success: Partner with the Outreach Specialist and Center Director to meet, and exceed, center enrollment goals through teamwork, communication, and creative problem-solving.
  • Promote Retention and Engagement: Support the Center Director by developing participant-retention strategies or serving as an active member of the retention committee.
  • Coordinate Care Efficiently: Request and track medical records from participant providers to ensure timely and accurate completion of the enrollment process.
  • Bring Our Mission to the Community: Attend outreach events alongside the Outreach Specialist to share our program and connect with potential participants and their families.
  • Ensure Accuracy and Compliance: Process enrollments promptly while maintaining complete and compliant documentation that meets all DHCS and CMS guidelines.
  • Contribute to Quality Excellence: Participate in PACE audits to support program integrity and continuous improvement.
  • Be a Welcoming Guide: Conduct initial phone screenings, lead engaging tours of our center, and perform home visits to complete the enrollment journey with care and compassion.
  • And of Course, Other Meaningful Work: Step in where needed to support our team and help participants access the care and community they deserve.

What Does An Ideal Candidate Look Like

  • A Heart for Service: Genuine passion for helping older adults live independently and with dignity in their communities.
  • Experience That Makes a Difference: Previous work in healthcare, senior services, or social services, especially in enrollment, outreach, or case management is highly valued.
  • Detail-Oriented and Organized: Strong ability to manage multiple tasks, track progress in CRM systems, and ensure documentation accuracy every step of the way.
  • Compliance-Minded: Familiarity with DHCS and CMS guidelines, or a willingness to learn quickly, so you can help uphold our program’s integrity and compliance.
  • Tech-Savvy and Adaptable: Comfortable using Microsoft Office, CRM platforms, and EMR systems to manage participant information efficiently.
  • Collaborative by Nature: Thrives in a team environment and enjoys partnering with Outreach Specialists, nurses, and other members of the Interdisciplinary Team.
  • A Confident Communicator: Excellent interpersonal and communication skills, with the ability to engage compassionately with participants, families, and care providers.
  • Reliable and Professional: Dependable, punctual, and able to maintain confidentiality while representing our program’s mission and values.
  • Bilingual a Plus: Spanish, Korean, Tagalog, or other languages commonly spoken in our communities are always welcome!

The Fine Print:

  • Your Schedule: Monday through Friday, with flexibility for occasional overtime or weekend events when community needs call for it.
  • Your Efforts Recognized: This role includes participation in an incentive plan that rewards the meaningful impact and hard work you bring to the team.
  • Your Work Environment: Though hybrid, this position is highly field-based and involves frequent travel throughout the PACE service area, sometimes up to 100%. You’ll spend much of your time meeting participants where they are, building relationships in their homes and communities.
  • Your Commitment to Safety: Driving is an essential part of the role, so a valid California driver’s license, reliable transportation, and proof of insurance are required.

The Physical Environment

  • On the Move: This role can involve up to 100% driving throughout the PACE service area. You will be out in the community visiting participants, providers, and event sites, sometimes in varying weather conditions such as heat, rain, or wind.
  • Active and Engaged: You must be able to walk independently between participant homes, community locations, and parking areas, often carrying materials or equipment needed for enrollment visits.
  • Community Facing: Expect frequent, meaningful interactions with participants, families, and community partners, listening, speaking, and engaging with warmth and professionalism throughout the day.
  • Tech Enabled: You will spend significant time on computers and mobile devices typing, entering data, sending emails, and documenting participant information accurately in real time.
  • Independent and Self Driven: While part of a supportive team, this position requires independence and adaptability as you plan your schedule, travel between sites, and balance both field and administrative work.
  • Blend of Field and Office Work: When not on the road, you will work from a hybrid or office setting, collaborating with your team, completing documentation, and preparing for upcoming enrollments.
