Job Title: Community Outreach & Medicaid Waiver Program Manager
Position Summary
We are seeking an experienced and driven Community Outreach & Medicaid Waiver Program Manager to lead and grow our Medicaid Waiver client acquisition and caregiver coordination efforts. This leadership role is responsible for overseeing outreach strategy, managing referral pipelines, strengthening community partnerships, and ensuring operational excellence in securing both clients and family caregivers.
The ideal candidate is a results-oriented leader with deep knowledge of the Medicaid Waiver process, a strong background in healthcare marketing or sales, and a proven ability to build and manage high-performing outreach initiatives.
Key Responsibilities
- Lead and oversee all community outreach and marketing efforts to drive Medicaid Waiver client referrals and admissions
- Develop and execute strategic plans to grow and sustain referral pipelines
- Build, manage, and strengthen relationships with hospitals, case managers, social workers, community organizations, and other referral sources
- Oversee the intake and coordination process to ensure family caregivers are secured efficiently for each referred client
- Collaborate with internal healthcare team members to ensure seamless client onboarding and service delivery
- Track, analyze, and report on outreach performance metrics, conversion rates, and growth opportunities
- Manage and improve processes related to Medicaid Waiver client acquisition and caregiver placement
- Represent the organization at community events, networking opportunities, and marketing initiatives
- Oversee and contribute to social media and digital outreach strategies to enhance brand presence and lead generation
- Identify and implement innovative and creative approaches to expand market reach and increase business growth
Leadership Expectations
- Provide direction, accountability, and oversight for outreach and intake functions
- Establish goals, KPIs, and performance standards for the department
- Identify gaps in processes and implement improvements
- Foster a collaborative, high-performance team environment
- Serve as a key liaison between leadership, clinical teams, and external partners
Qualifications
- Minimum 5+ years of experience in sales, marketing, or business development, preferably in healthcare
- Proven track record of successfully generating Medicaid Waiver client referrals
- Strong knowledge and hands-on experience with the Medicaid Waiver process
- Demonstrated leadership or management experience preferred
- Excellent communication skills, both verbal and written
- Strong relationship-building and networking abilities
- Highly organized with strong attention to detail and follow-through
- Proficient in medical terminology, documentation, and healthcare workflows
- Experience with social media platforms and marketing strategies
- Strategic thinker with a proactive and creative mindset
