We are a Federally Qualified Health Center (FQHC) that provides Article 16 and 28 clinic services at 8 locations across Suffolk County, New York. We are a multi-specialty healthcare provider caring for our community through Primary and Specialty Medical Care, Dental, Behavioral Health, Audiology, Physical Therapy, Occupational Therapy and Speech Language Pathology. We currently provide care to over 7,000 patients and have an annual operating revenue of $22m.

About the Role:

As a Nurse Practitioner in Smithtown, NY, you will play a crucial role in delivering high-quality healthcare to patients across various age groups, including pediatrics and geriatrics. Your primary responsibility will be to conduct comprehensive clinical assessments, develop treatment plans, and manage pain effectively, ensuring patient safety and well-being. You will collaborate with a multidisciplinary team to provide holistic care, addressing both physical and emotional health needs. This position requires a strong commitment to patient education and advocacy, empowering individuals to make informed health decisions. Minimum Qualifications:

  • Master's degree in Nursing with a specialization as a Nurse Practitioner.
  • Current Nurse Practitioner license in the state of New York.
  • Certification in Family Medicine or a related specialty.

Preferred Qualifications:

  • Familiarity with electronic health record (EHR) systems.
  • Bilingual skills to communicate effectively with diverse patient populations.

Responsibilities:

  • Conduct thorough clinical assessments and evaluations of patients to determine their healthcare needs.
  • Develop and implement individualized treatment plans, including pain management strategies.
  • Provide preventive care and health education to patients and their families.
  • Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care.
  • Monitor patient progress and adjust treatment plans as necessary, maintaining accurate medical records.
