Responsibilities:
- Conduct comprehensive assessments of patients' physical abilities and limitations.
- Develop and implement individualized therapy plans to address specific conditions and enhance physical function.
- Provide direct therapy services to clients in individual and group settings.
- Educate patients and their families on techniques to improve physical function and prevent injury.
- Monitor patients' progress and adjust therapy plans as needed to promote optimal outcomes.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
- Document patient care accurately and maintain confidentiality of patient information.
- Provide emotional support and counseling to patients and their families to promote well-being and coping strategies.
- Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.
Physical Therapist Qualifications:
- Current Physical Therapist (PT) license in the state of CA.
- 1-2 years of clinical experience in physical therapy, preferably in a similar setting.
- BLS/CPR certification required.
- Valid CA Driver’s license.
Documentary Requirements:
- Compliance with federal I-9 requirements (please bring suitable documentation with you on your first day of work verifying your identity and legal authorization to work in the United States); and Mandatory Physical (within the last 6 months prior to the orientation date). You may bring a note from your physician prior to the Orientation Date.
- Mandatory TB (within the last year) OR Chest X-ray (within the last 5 years).
- MMR immunization or titer showing immunity prior to your Orientation Date.
- Varicella immunization or titer showing immunity prior to your Orientation Date.
- Hep B immunization or titer showing immunity prior to your Orientation Date.
- TDAP vaccination or declination prior to your Orientation Date.
- Yearly Influenza vaccination or declination prior to your Orientation Date.
