As a major regional employer, the organization operates hospitals, outpatient clinics, rehabilitation centers, and a broad continuum of health and social services.
This Physical Therapist role is based in an outpatient rehabilitation clinic and focuses on providing high-quality physical therapy services to patients with a wide range of diagnoses and functional limitations. The position emphasizes patient-centered care, interdisciplinary collaboration, and evidence-based treatment approaches.
Key Responsibilities
Provide skilled physical therapy services to patients with diverse diagnoses, disabilities, and functional needs.
Conduct comprehensive patient evaluations and develop individualized treatment plans based on clinical findings and patient goals.
Deliver therapeutic interventions to promote functional independence and maximize patient outcomes.
Adapt treatment approaches to account for age-related, medical, and functional considerations.
Collaborate with physicians and interdisciplinary care teams to coordinate patient care.
Participate in professional consultations to support optimal rehabilitation outcomes.
Maintain accurate, timely, and compliant clinical documentation.
Adhere to organizational policies, professional standards, and regulatory requirements.
Required Qualifications
Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) degree from an accredited Physical Therapy program.
Active Physical Therapist license in the state of practice upon hire.
Ability to obtain licensure in additional states where care may be delivered, if applicable.
Current BLS certification through the American Heart Association (required upon hire).
Willingness to complete specialized training if required to meet departmental needs.
Preferred Qualifications
Prior experience in an outpatient rehabilitation setting.
Specialized physical therapy experience aligned with departmental needs.
