Job Title: Hospice Clinical Liaison – Registered Nurse (RN)
Pay: $90,000 – $130,000 per year DOE + Full Benefits Package
Schedule: Monday – Friday with on-call and weekend rotation
Location: Twin Cities Metro | 21-County Service Area

Our team focuses exclusively on hospice and palliative services, allowing us to provide highly personalized care to patients and families wherever they reside.
We are currently seeking an experienced Registered Nurse to serve in a Clinical Liaison role, supporting referrals, admissions, facility relationships, and clinical coverage across the organization. This position is ideal for an RN who enjoys a fast-paced environment, working with referral partners, and playing a key role in helping patients access hospice services at the right time.

Key Responsibilities:
- Serve as a clinical liaison with hospitals, SNFs, assisted living, and referral sources.
- Review referral reports and coordinate with intake on new hospice opportunities.
- Complete hospice admissions including assessments, consents, education, and start of care.
- Conduct informational visits with patients and families to explain hospice services.
- Assist with visit coverage when needed to support case managers.
- Communicate with physicians, discharge planners, social workers, and families.
- Help maintain and grow referral relationships across the service area.
- Ensure documentation meets Medicare hospice regulations.
- Provide clinical support wherever needed to maintain continuity of care.
- Travel throughout the Twin Cities metro and surrounding counties.
Qualifications:
- Active Minnesota RN license required.
- Hospice or tangible end of life care experience.
- Will consider ICU, oncology, palliative care, ER, or other high-acuity backgrounds.
- Experience with hospice admissions strongly preferred.
- Comfortable working independently in the field.
- Strong communication and relationship-building skills.
- Ability to manage multiple priorities in a fast-moving environment.
- Reliable vehicle and ability to travel within the service area.
