35-hour, 102-bed skilled nursing center located near Riverside Regional Medical Center and a proud member of the Consulate Health Care family.

$39-$40 per hour

Responsibilities

  • Provide comprehensive occupational therapy assessments and develop client-centered treatment plans.

  • Deliver evidence-based OT interventions to improve activities of daily living and functional independence.

  • Monitor client progress with measurable outcomes and adjust plans accordingly.

  • Collaborate with multidisciplinary teams to coordinate care and ensure seamless service delivery.

  • Document clinical notes, treatment plans, and progress in electronic health records.

  • Identify and escalate safety concerns; implement risk management and infection control practices.

  • Educate clients and families on therapeutic goals, exercises, and home-programs.

  • Contribute to pipeline and territory planning to optimize patient referrals and outcomes.

  • Demonstrate professional development through ongoing learning and application of new skills.

  • Participate in performance reviews and set individual targets aligned with company metrics.

  • Maintain up-to-date knowledge of OT best practices, guidelines, and regulatory requirements.


Experience/Qualifications

  • Valid OT license and current state registration with no active sanctions.

  • Doctor of Occupational Therapy (OTD) or equivalent; minimum 2 years OT experience.

  • Demonstrated experience in comprehensive OT assessments and individualized treatment plans.

  • Evidence-based intervention delivery and measurable outcomes for ADLs and independence.

  • Proficiency in documenting in electronic health records with timely, accurate notes.

  • Strong collaboration with multidisciplinary teams and care coordination skills.

  • Risk management, infection control, and safety escalation protocols.

  • Commitment to ongoing professional development and knowledge of OT guidelines/regulations.

  • Excellent communication skills for patient/family education and home-program guidance.

  • Performance-driven with ability to использовать pipeline data for territory planning and referrals.
