Director Of Case Management-Tampa, FL
Schedule: Full-time • Days (No Weekends)
Job Summary:
The Case Management Director is responsible for managing and coordinating all department activities. This role ensures staff compliance with organizational policies and external regulatory requirements while leading the integration of patient care and discharge planning processes across hospital departments and external agencies.
The Director oversees daily case management operations and is accountable for achieving established outcomes by actively engaging interdisciplinary teams and stakeholders. This position requires strong leadership presence, excellent communication and presentation skills, and a proven track record of achieving results.
Job Responsibilities:

Direct and evaluate departmental operations, including case management model, staffing (skill mix and FTEs), technology use, onboarding, and staff competencies

Allocate resources to meet productivity and quality goals

Oversee core case management functions and practices

Ensure adherence to care coordination and discharge planning processes, including timeliness, quality, and documentation

Collaborate with leadership to meet organizational goals

Monitor and improve departmental performance through data analysis and process improvements

Ensure compliance with policies and standard operating procedures

Build and maintain relationships with key stakeholders across clinical and administrative teams
Job Qualifications:

Bachelor's degree - Required

Master's degree (Nursing, Health Administration, or Business Administration) - Preferred

Active licensure as RN, LCSW, or LMSW - Required

3+ years of acute care hospital case management experience - Required
-
2+ years of case management leadership experience in acute care - Required
