Organization Profile: Hawthorne Foundation Inc. serves children and adults with autism and other developmental disabilities. Our educational, vocational and life skills programs are only a few of the programs available. We also offer parent training, recreational activities and crisis intervention.

Mission: An Applied Behavior Analysis approach to lifelong quality educational, habilitative, vocational and recreational programs for individuals with autism and other intellectual disabilities.

Our goal is to enable each and every individual to reach his/her maximum potential in the least restrictive environment by selecting and promoting educational practices that are grounded in research and science, in particular applied behavior analysis; and, to assist each individual with identifying and realizing his/her own goals.

Qualifications:

New York State License Occupational Therapist (OTRL), Speech Therapist (SLP), Physical Therapist (DPT) or Licensed certified social worker (LSCW)

Master’s Degree from a nationally accredited college or university.

An ability to provide direct therapy services to students with disabilities.

Sensitivity to the communication, academic and social needs of the students with developmental disabilities at all levels of independence.

Responsibilities:

Direct treatment sessions twenty-five sessions per week.

Timely Daily completion of session notes.

Completion of Daily CM 2/attendance forms.

End of Week reviews of all notes and cm 2

Timely monthly submission of CM 2 forms with self-audit submitted according to assigned dates to the IEP specialist.

Monthly self-audit of session notes: Self audit form completed by therapist with corrections or missing notes completed and signed.

Daily session notes completed within five days of service provision. Completed in IEP direct.

Timely Completion of Quarterly Progress reports completed according to scheduled periods set by HCDS in IEP direct.

Senior teachers print out all reports and send them home to the parents.

Completes annual reports for the current school year and set goals for the following school year in IEP direct.

Annual reports and goals are added to the share drive in the student folder. Contacts parents to collaborate and develop new goals. Completed reports and goals and adds to the student file in the share drive. One copy sent to the parents, and one copy goes to the IEP specialist. All reports and goals are due two weeks prior to scheduled meetings.

Timely completion of summer reports in IEP direct.

Ensures all documentation is compliant with all regulations including Medicaid.

Addresses the instructional programming/curriculum development to address therapy needs of School Age students.

Attend parent and staff meetings and in-services offered by Hawthorne Country Day as and when requested

Works with HR department and contracted agencies to fill Speech/OT/PT/Counseling vacancies and ensures IEP mandates are fulfilled.

Interviews with RS therapy candidates.

Supervised COTAs to meet Oversight and Supervision requirements with NYSED, NYS Office of Professions and Medicaid.

Arranges for Oversight and Supervisions by RS PT/Speech/Counseling staff to meet necessary state requirements; NYSED, NYS Office of Professions and Medicaid.

Create caseloads with IEP direct specialist and schedules for contract therapists as needed.

Monthly review of departmental daily note/CM2 for SP/OT/PT/Counseling department.

Ensures submission of completed daily notes and CM2 billing across all Speech/OT/PT/Counseling therapists on the last day of the month.

Ensures therapists complete and submit Speech/OT/PT/Counseling Quarterly/Annual CM5 Progress Reports, annual review packets, goals, and regression summaries, and self-audits. • Ensures therapists are compliant with all due dates for session notes and reports.
