Position Overview

The Associate Client Care Manager (ACCM) is a vital member of Care Indeed’s operations team, crucial in ensuring high client retention and caregiver retention by ensuring exceptional care experiences and strong relationships, and comprehensive support in care coordination. This role encompasses a wide range of responsibilities, including managing client care plans, coordinating care services, conducting walk-in assessments, and participating in retention initiatives. The ACCM must possess strong communication and problem-solving skills, enabling them to convey care plans to caregivers effectively, monitor multiple client accounts, and build lasting relationships with clients and care providers.

By facilitating new client onboarding and ongoing care coordination, the ACCM helps maintain exceptional client experiences and the highest standards of care. The ACCM's dedication to continuous improvement and maintaining great relationships between Care Indeed and its clients is instrumental in achieving the agency's overall mission of providing exceptional home care services.


Essential Duties And Responsibilities

  • Build and maintain strong relationships with clients and care providers, ensuring their needs and preferences are consistently met, and any concerns are promptly addressed.

  • Monitor the quality of care provided to clients by conducting regular assessments, reassessments, client and caregiver feedback sessions, and care conferences.

  • Develop and implement strategies to improve client retention, including personalized care plans, regular communication, and timely resolution of any issues.

  • Collaborate with the operations manager and care providers to continuously improve the quality of care, identifying areas for improvement and implementing best practices.

  • Conduct regular quality assurance audits, including reviewing care documentation, client records, and care provider performance evaluations.

  • Coordinate with the operations manager to ensure compliance with all company policies, procedures, and regulatory requirements related to client care, safety, and privacy.

  • Participate in ongoing professional development and training opportunities to stay current on industry best practices and enhance leadership skills.

  • Facilitate engagement, outreach, and marketing opportunities as requested by the supervisor.

  • Assist in caregiver orientation and training.

  • Provide extra support for new client cases, including home visits during the initial care visits, and serve as a backup as needed.

  • Successful staffing supervision and coordination of care

  • Create and maintain referral resources to achieve the referral sources' satisfaction.

  • Additional responsibilities may be assigned as the need arises, and the employee will be notified, and the manager or HR will hold a meeting to discuss the changed responsibilities.


Job Conditions

This job operates in a professional office environment. This role routinely uses standard office equipment such as laptops, smartphones, photocopiers/scanners, shredders, and filing cabinets. Risk of exposure to bloodborne pathogens-limited

  2. The position is stressful in terms of meeting deadlines.

  3. It requires minimal lifting of office records and printouts.

  4. It is primarily a desk job that essentially involves sitting, standing, stooping, and walking, as well as an inordinate amount of telephone communication.

  5. Travel is required, by car to local, out-of-town seminars, conferences, or meetings


Qualifications

  • Bachelor's degree in psychology, counseling, rehabilitation, gerontology, nursing, or sociology required.

  • Training in gerontology/geriatrics preferred

  • Background in behavioral health, psychology, or related field preferred.
  • Minimum of 2 years healthcare experience preferred - prior client management/or marketing experience in the health/home care industry a plus.

  • Working knowledge of Microsoft Office (Word, Excel, PowerPoint, and Outlook)

  • Availability during and after business hours and on-call rotation on weekends.

  • Have strong communication skills, both written and verbal, and a willingness to learn.

  • Ability to incorporate the ethical work standards in all aspects of interactions with clients and employees

  • Strong interpersonal and communication skills to establish rapport with clients and their families, as well as to collaborate effectively with the operations team.

  • A valid driver's license, reliable transportation, and willingness to travel within the assigned territory.

  • Commitment to providing exceptional client care and support, as well as a strong interest in professional growth and development.


Work Hours

Monday-Friday 9:00 AM to 5:30 PM (days may change depending on the need) and occasional weekends as scheduled ahead of time as needed.
