Job Title: Registered Nurse – Case Manager
Location: Buckley AFB Health Clinic, Colorado
Reports To: TBD
Schedule: Monday – Friday, 0730 to 1630 (9-hour shifts with a 60-minute unpaid lunch)
Position Summary:
The Registered Nurse – Case Manager (RN-CM) provides comprehensive case management services to patients at Buckley AFB Health Clinic. The RN-CM plays a critical role in coordinating patient care across the healthcare continuum, ensuring that services are efficient, high-quality, and tailored to meet individual needs. This position requires a thorough understanding of nursing, case management principles, and healthcare systems.
Qualifications:

Education: Associate Degree in Nursing from a program accredited by ACEN or CCNE.

Licensure/Certifications: Must possess one of the following case management certifications:

CCM, CDMS, CRRN, COHN, COHN-S, ACCC, CRC, RN-NCM, or CMC.

Experience: Minimum of one year of professional nursing experience.
Key Responsibilities:

Manage all phases of the Case Management Program (CMP) in alignment with established CM standards of care.

Deliver expert nursing input in care assessment, planning, implementation, coordination, and monitoring.

Identify and act on case management opportunities in both inpatient and outpatient settings, including telehealth.

Develop tools and strategies to support case management such as patient assessments, clinical guidelines, and resource databases.

Integrate nursing case management with utilization management (UM) and social work services.

Establish and maintain relationships with community agencies and organizations to support patient care.

Document patient care data accurately and in a timely manner.

Develop and coordinate individualized treatment plans that include preventive, therapeutic, rehabilitative, and psychosocial interventions.

Ensure continuity of care through appropriate discharge planning and follow-up.

Educate patients and caregivers based on identified healthcare needs.
This position supports the healthcare readiness and well-being of military personnel and their families, contributing to a collaborative and mission-focused care environment.
