MDS Coordinator (RN/LPN) – Skilled Nursing Facility
What You’ll Do:
<ul>
<li>
Complete and submit MDS assessments on time
</li>
<li>
Coordinate care planning with the interdisciplinary team
</li>
<li>
Ensure documentation supports PDPM and reimbursement accuracy
</li>
<li>
Educate staff on MDS and care planning requirements
</li>
<li>
Support audits, quality assurance, and compliance
</li>
</ul>
What We’re Looking For:
<ul>
<li>
Active RN or LPN license (required)
</li>
<li>
Skilled nursing/long-term care experience preferred
</li>
<li>
Knowledge of MDS 3.0, RAI process, and PDPM
</li>
<li>
Strong attention to detail, organization, and teamwork skills
</li>
</ul>
Schedule: Full-time, Monday–Friday
