Patient Advocate (Hospice Liaison) – Ovation Hospice | Utah

We are looking for Patient Advocates who are driven, self-directed, and genuinely moved by this work. This is a field-based relationship role: you will be out in the community every day, building trust with physicians, hospital discharge teams, skilled nursing facilities, and case managers — and serving as the bridge between families navigating an incredibly difficult time and the care they deserve.
This is not a transactional sales position. It requires someone who can hold both the relational and the professional simultaneously — who understands how to build a referral territory from the ground up, stay accountable to their own numbers, and show up with warmth and credibility in environments where the stakes are high.
Cohort hiring note: We are currently hiring this role as a cohort across all three markets simultaneously. Selected candidates will share the same start date, onboarding experience, and compensation structure. We are building this team the right way — together.
What you will do
- Build and manage a referral territory — cultivating relationships with physicians, hospital case managers, SNF social workers, and other healthcare partners
- Educate referral sources on Ovation's clinical capabilities, patient criteria, and the value of timely hospice referrals
- Meet with families to explain hospice benefits and help them understand their options during an emotionally complex time
- Drive admissions growth by consistently generating qualified referrals and converting them to patients in care
- Maintain accurate activity tracking in CRM — logging visits, contacts, and pipeline with discipline
- Collaborate closely with your Administrator, clinical team, and intake coordinators to ensure seamless hand-offs and patient experience
- Represent Ovation in the community with professionalism, humility, and genuine care for the people you serve

Prior experience in hospice, home health, healthcare sales, case management, or a community liaison role is strongly preferred. We will also consider candidates from outside healthcare who demonstrate exceptional relational drive and a genuine connection to this work.

Our hiring process
