BEHAVIORAL HEALTH CLINICIAN - MC3

The general supervision of the Behavioral Health Consultant/Clinician is provided by the DWIHN Outpatient Clinic Services Director or designee. The Michigan Child Collaborative Care Program (MC3) and Behavioral Health Consultant Project provides behavioral health consultation for local primary care providers with MC3 child(ren), adolescent(s) and prenatal psychiatrists. The DWIHN Outpatient Clinic provides local oversight, in collaboration with MC3 program, of the Behavioral Health Consultant as they implement MC3 in Wayne County as well as work in concert with other regional Behavioral Health Consultants.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

The Behavioral Health Consultant/Clinician:

  1. Facilitates consultation between Provider and MC3 consulting specialists.
    • Provides first-line, phone coverage to enrolled providers and in MC3 designated catchment area and provides back-up coverage for other MC3 regions when needed.
    • Assists in triaging identified children, adolescents, and perinatal women with various psychiatric conditions to phone-based or telemedicine consultation with MC3 or local community-based services.
    • Facilitates individual and group consultation between providers and MC3 consulting specialists in healthcare and school settings.
    • Recruits new providers throughout the year.
    • Assists the provider and MC3 in the coordination of follow up activities.
    • Facilitates information sharing between providers and MC3 psychiatrists and local providers.
    • Facilitates telemedicine evaluations.
  2. Facilitates coordination of care for children, adolescents, and perinatal women.
    • Provides resource information within identified region for children, adolescents and perinatal women based on recommendations by the MC3 consulting psychiatrist. Coordination may include referrals for psychotherapy, school-based services, or other community-based services.
    • Champions regional resources and knowledge of local CMH agencies access and programs to provide accurate and validated resource recommendations to patients and providers.
  3. Provides behavioral consultation services in designated sites (for sites with embedded BHCs only)
    • Provides brief mental health assessments to assist with referrals to appropriate providers, as indicated.
    • Works with healthcare team on developing workflow for same-day and scheduled referrals to MC3 and follow-up on screening.
    • Provides brief interventions to children, adolescents and their families and/or perinatal women. Brief interventions may include anticipatory guidance, psycho-education, motivational interviewing, information on services available to address identified concerns or other approaches as appropriate for age and disposition.
    • Participates in meetings with healthcare team to discuss children, adolescents and/or perinatal women who are served in the practice.
    • Completes face-to-face, virtual or phone-based follow-up with patient/family to help the providers monitor adherence to treatment and ensure linkage to resources.
    • Completes follow-up outcomes measures with patient/family to help monitor progress toward treatment goals.
    • Completes patient follow-up surveys post intervention.
    • Participates in panel reviews with consulting psychiatrist.
  4. Assists with data collection and project evaluation.
    • Collects evaluation data on children, adolescents, and perinatal women referred by providers as determined by MC3 guidelines.
    • Ensures data is entered into the secure web-based system according to MC3 standards.
    • Submits data on activity for MC3 statistical and funding reports.

  • Knowledge of the clinical care process (screening, assessment, treatment planning, case management, and continuing care).
  • Knowledge of and ability to use screening and assessment tools for behavioral health services.
  • Knowledge of the continuum of care for all disability designations (SED, SMI, Co-Occurring Disorder).
  • Knowledge of Outpatient and Co-Occurring services.
  • Knowledge of the Open Access Model.
  • Knowledge of Integrated Care services for behavioral health patients.
  • Knowledge of Medical Necessity Criteria for Behavioral Health Services.
  • Knowledge of the Federal Confidentiality Regulations, 42 CFR, Part 2.
  • Knowledge of Motivational Interviewing.
  • Knowledge of documents / regulations that govern the provision of mental health services, e.g., Medicaid Manual Mental Health and Substance Abuse Chapter III, State Plan for Medicaid, Michigan Department of Health and Human Services Quality Plan, BBA requirements and the Mental Health Code.
  • Knowledge of the practices and principles of psychological, emotional, and sociological assessment and diagnosis.
  • Knowledge of crisis training and experience in de-escalation, i.e., suicide prevention, development of crisis plans, trauma informed care, etc.
  • Knowledge of diagnostics, psychopharmacology and supportive treatment approaches.
  • Knowledge of the identification and treatment of co-occurring mental health and substance use disorders.
  • Knowledge of community mental health services and resources.
  • Knowledge of the DSM-V Statistical Manual.

REQUIRED EDUCATION:

  • A Master’s Degree from a recognized college or university in the Human Services, the Social Services, Psychology, Counseling, Social Work or a related field.

REQUIRED EXPERIENCE:

  • Three (3) years of professional clinical experience in a behavioral healthcare or primary care setting, preferred.
  • At least two (2) years of clinical experience working with children and/or youth.
  • MichiCANs certification preferred or must be willing to complete the training and become certified.
  • Must be able to be credentialed with: QMHP and CMHP

REQUIRED LICENSE(S):

  • A Valid State of Michigan clinical licensure: LMSW, LMHC, LPC, LLP or PhD.
  • A valid State of Michigan Driver’s License with a safe and acceptable driving record.

WORKING CONDITIONS:

This position will primarily work at the designated site. This position will be required to work with persons who may suffer from mental health or substance abuse crisis.
