Inabia is seeking a compassionate and motivated Physical Therapist to join our Rehab Therapy team in Alaska. This role focuses on providing high-quality home health physical therapy services to patients within Anchorage and Eagle River. The ideal candidate will be patient-focused, clinically skilled, and comfortable working independently in a home health setting.
Key Responsibilities
<ul>
<li>
Provide skilled physical therapy services to patients in their homes in accordance with physician orders and established care plans
</li>
<li>
Evaluate patients’ functional abilities, mobility, strength, and balance
</li>
<li>
Develop and implement individualized treatment plans to restore function and improve quality of life
</li>
<li>
Educate patients and caregivers on exercises, safety, and home programs
</li>
<li>
Accurately document visits, progress notes, and discharge summaries in compliance with company and regulatory standards
</li>
<li>
Coordinate care with physicians, nurses, and interdisciplinary team members
</li>
</ul>
Schedule &amp; Workload
<ul>
<li>
Shift: Monday–Friday, 8:30 AM – 5:00 PM
</li>
<li>
Visits: Average of 5–6 patient visits per day
</li>
<li>
Start Date: Flexible; must be able to start no later than February 16
</li>
