The Full-Time Nurse Manager plays a critical leadership role in ensuring the delivery of safe, effective, and compliant home health care services. This position oversees the daily clinical operations, supervises nursing staff, ensures adherence to all regulatory requirements, and works collaboratively with caregivers, patients, families, and interdisciplinary teams to achieve optimal patient outcomes.
Key Responsibilities
- Supervise, mentor, and support field nurses and other clinical staff, providing ongoing education, performance evaluations, and guidance.
- Oversee patient admissions, care planning, and case management to ensure high standards of care.
- Monitor and ensure timely completion of OASIS documentation, Plan of Care, and 485s.
- Conduct regular chart reviews and home visits to maintain quality assurance and compliance with state, federal, and agency policies.
- Collaborate with physicians, caregivers, and families to coordinate individualized care plans.
- Monitor patient progress and adjust care plans as needed.
- Participate in the recruitment, onboarding, and retention of qualified nursing staff.
- Serve as a resource for staff regarding complex clinical issues.
- Assist in developing and implementing policies and procedures to meet regulatory standards and support best practices.
- Support the Administrator with surveys, audits, and compliance initiatives.
- Provide staff education and ongoing training related to compliance and quality standards.
- Maintain current knowledge of home health industry trends, regulations, and best practices.
- Participate in the agency’s QAPI program.
- Participate in on-call rotation as needed.
Qualifications
- Current and unrestricted Registered Nurse (RN) license in CT.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 5-7 years of nursing experience, with at least 3-5 years in home health.
- 1- 2 years of prior supervisory or management experience required.
- Strong knowledge of CMS CoPs and state home health survey requirements
- Excellent leadership, organizational, and communication skills.
- Strong critical thinking and problem-solving abilities.
- Strong Knowledge in OASIS
- Ability to travel locally to patient homes as needed; valid driver’s license and reliable transportation required.
- Proficiency with Home Care Home Base or similar (EMR) systems and Microsoft Office Suite.
