Program Highlights:

Participants will engage in a paid, immersive 6-week onboarding program that merges hands-on learning with guidance from seasoned Case Management professionals. The program also offers 43 hours of Continuing Education Units (CEUs) applicable to your nursing license.

To further facilitate your professional development, the organization covers the costs of the American Case Management Association (ACM) certification exam and all necessary study materials, eliminating financial barriers and investing in your growth from the outset.

In your capacity as a Case Manager, you will assist patients and their families throughout their care journey. Your responsibilities will include coordinating services, connecting resources, and crafting care plans tailored to each patient’s unique needs. By collaborating with physicians, nurses, and department leaders, you will help ensure safe transitions, effective communication, and consistent quality across the care continuum.
Your key responsibilities will encompass:
- Guiding patients and families through program orientation, detailing the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
- Coordinating educational efforts for patients, families, and caregivers to foster involvement in treatment planning, goal discussions, and family conferences.
- Creating and executing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery objectives.
- Conducting psychosocial assessments and discharge planning evaluations, thoroughly documenting findings and communicating needs across the care team.
- Working closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.

Qualifications Required:
- Registered Nurse (RN) license
- Bachelor's Degree, Master's Degree, or Registered Nurse Diploma
- No travel required
- Minimum of 3 years of relevant experience
