Unit Description – Case Management Department
Type of Care:
- Focused on care coordination and discharge planning for medical and surgical inpatients.
- Ensures safe and timely transitions from hospital to post-acute settings.
- Promotes patient advocacy, resource management, and continuity of care.
Patient Population:
- Adult and geriatric patients from various specialties including medical, surgical, ICU, and women’s services.
- Includes patients with complex psychosocial or discharge needs (e.g., homelessness, substance use, mental health concerns).
Average Caseload:
- Up to 25 patients per Case Manager.
Shift & Schedule:
- 8-hour shifts: 08:00 AM – 04:30 PM.
- Schedule varies to meet unit needs; 4-week rotation.
- Float and weekend coverage required (every other or every third weekend).
- Call coverage as needed.
Staffing Ratios:
- Day Shift: 1:25
- No LVN, Tech, or CNA assigned to Case Management.
Core Responsibilities:
- Perform comprehensive assessments, develop and implement discharge plans.
- Coordinate with interdisciplinary teams, physicians, and families.
- Arrange DME, SNF placements, rehab transfers, and ambulance transport independently.
- Address barriers to discharge including insurance, housing, and psychosocial needs.
- Facilitate family meetings and ensure documentation compliance.
- Work closely with HMOs, IPAs, and community resources.
Documentation & Systems:
- Epic (HARDSTOP) for documentation.
- SBAR communication.
- Pyxis for medication system reference.
Support Services Available:
- Pharmacy, Radiology, Respiratory Therapy, Physical Therapy, Social Services, Transportation, and Interpretation Services.
- In-house Hospitalist coverage 24/7.
Special Notes:
- Case Managers do not perform Utilization Review.
- Epic experience required.
- Strong knowledge of community resources, protective services, and post-acute care coordination is essential.
Certifications:
- BLS – Required
Experience Requirements:
- Minimum 2 years acute medical Case Manager experience performing care coordination and discharge planning.
- Must be competent with DME, SNF, and transport arrangements.
- Travel experience required.
- Behavioral health-only backgrounds not accepted.
Orientation & Additional Info:
- Orientation: 40 hours.
- Scrubs: Turquoise, provided by facility.
- Weekend rotation required; caseload up to 25 patients.
- Limited pre-approved time-off (1–2 requests per contract).
