Responsibilities

  • Provide direct patient care under supervision of registered nurse or physician.

  • Administer medications and monitor patient responses.

  • Collaborate with healthcare team to ensure optimal patient outcomes.

  • Monitor and report changes in patient conditions to healthcare team.

  • Document patient information accurately and maintain medical records.

  • Educate patients and families on health management and procedures.

  • Ensure compliance with healthcare regulations and standards.

  • Support team in achieving performance targets and goals.

  • Assist in developing and implementing patient care plans.

  • Participate in continuous professional development and training.

  • Maintain a clean and safe working environment.

  • Utilize pipelines to assess and improve patient care strategies.


Experience/Qualifications

  • Valid LVN/LPN license in the state of employment.

  • Excellent communication skills for patient and family education.

  • Ability to work in a results-oriented environment with high visibility of work activity.

  • Strong ability to administer medications and monitor patient responses effectively.

  • Proficiency in documenting patient information and maintaining medical records.

  • Demonstrated ability to collaborate with healthcare teams for optimal outcomes.

  • Commitment to continuous professional development and training.

  • Knowledge of healthcare regulations and standards compliance.

  • Experience in developing and implementing patient care plans.
