What We're Looking For
✔ Active RN License
✔ Minimum 2 years of Home Health Management experience REQUIRED
✔ Strong knowledge of Medicare Conditions of Participation (CoPs)
✔ Experience leading and developing clinical teams
✔ HomeCare HomeBase experience preferred

Key Responsibilities

• Manage daily clinical operations and staff performance
• Ensure compliance with federal, state, and agency regulations
• Oversee patient care planning, quality assurance, and QAPI activities
• Support and mentor clinicians to achieve outstanding patient outcomes
• Collaborate with interdisciplinary teams to ensure continuity of care
• Occasional visits may be required.
