About the Role:
As a Physical Therapist in our Health Care Services team based in Richardson, TX, you will play a critical role in improving the physical health and functional mobility of patients across diverse age groups. Your primary objective will be to assess, plan, and implement individualized rehabilitation programs that address orthopedic conditions, sports injuries, and geriatric rehabilitation needs. You will collaborate closely with physicians, occupational therapists, and other healthcare professionals to ensure comprehensive patient care and optimal recovery outcomes. This role demands a compassionate approach to patient interaction, combined with expert knowledge in manual therapy and sports medicine to facilitate effective treatment. Ultimately, your work will contribute significantly to enhancing patients' quality of life by restoring their physical independence and reducing pain or disability.
As a Physical Therapist, you will:

Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

Applies concepts of infection control/universal precautions in performing patient care

Assure continuity of quality patient care delivered with appropriate documentation

Provide effective communication to patient/family, team members and other health care professionals

Promote agency philosophy and administrative policies

Maintain and complete all documentation within company stated guidelines.
Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Valid Physical Therapy license
-
1 year of home health experience preferred
-
CPR certification
-
Valid Driver’s license
-
Valid Auto Insurance
-
HomeCare HomeBase experience preferred
