Adecco Healthcare & Life Sciences is hiring a Case Manager in Sacramento, California! This role is in person / onsite. Please read below and apply with an updated resume for consideration.
Type: Six-month contract, opportunity for extension or full-time offer
Schedule: Monday through Friday, 8am to 5pm
Pay: $27.00 to $30.00 an hour
Responsibilities of the Case Manager
· Screen prospective participants by verifying income and documentation, obtaining necessary consents, and reviewing eligibility with supervisors.
· Conduct comprehensive assessments (HMIS, VI-SPDAT, Housing Retention) to evaluate participant strengths and risks while providing full orientation on program services, rights, and responsibilities.
· Conduct interviews and research to identify risks, stratify needs, and develop data-driven care plans.
· Establish participant-driven goals based on resource availability, consensus, and required level of care.
· Provide financial coaching—including budgeting, tax filing (EITC), and credit repair—to support long-term stability.
· Link participants to internal programs and external community resources, ensuring minor children are enrolled in school.
· Maintain a caseload of 20 households, facilitating case conferences and acting as a liaison between service partners and participants.
· Perform routine reassessments and proactively adjust care plans to improve outcomes in static or regressive situations.
· Manage case closures and exits—both voluntary and involuntary—ensuring all agency procedures and adverse action protocols are followed.
· Keep all relevant service data up to date in HMIS system and maintain case files in accordance with applicable guidelines, completing data entry immediately or no later than within 24 hours of service interaction.
Qualifications of the Case Manager
· High school diploma or GED required
· 1+ years’ experience working with low income and/or homeless population.
· Associates or Bachelor’s degree in lieu of experience will be highly considered
· Proficient computer skills, including Microsoft Office Suite (Outlook, Word and Excel)
· Experience working in diverse settings with people across all socio-economic spectrums and a wide variety of personalities and roles - staff, residents, local agencies, contractors, lenders, etc.
· Valid driver’s license and clean driving record
