Job Summary

Lifekind Health is seeking an experienced and compassionate Registered Nurse Lead (RN) to oversee clinical operations and care management services within one clinic location. This role combines hands-on patient care with clinical leadership, staff supervision, and oversight of Enhanced Care Management (ECM) and case management services.

The RN Lead will supervise and support Licensed Vocational Nurses (LVNs), Community Health Workers (CHWs), and care coordinators while also actively participating in patient care, care planning, and complex case management. This position is ideal for a strong clinical leader who is organized, collaborative, and knowledgeable in population health and ECM workflows.

Key Responsibilities

Clinical Leadership & Operations

  • Provide day-to-day clinical leadership and supervision for LVNs, Community Health Workers, and care coordinators within the clinic
  • Serve as the clinical point of contact for staff questions, escalations, and complex patient needs
  • Collaborate closely with providers, behavioral health teams, and clinic leadership to ensure coordinated, high-quality care
  • Support clinic workflow optimization, patient flow, and interdisciplinary communication

Enhanced Care Management (ECM) & Case Management

  • Oversee and support ECM and case management services in alignment with payer (e.g., IEHP) and regulatory requirements
  • Provide clinical oversight for high-risk and complex patients, including care planning, risk stratification, and care transitions
  • Ensure appropriate documentation, compliance, and reporting for ECM and case management activities
  • Guide and support CHWs and care coordinators in ECM workflows, outreach, engagement, and escalation processes
  • Act as the primary clinical escalation point for complex cases, hospital discharges, TOCs, and care coordination needs

Direct Patient Care

  • Provide direct nursing care to patients as needed, including assessments, patient education, care coordination, and clinical support
  • Participate in patient visits and interdisciplinary care planning when clinically appropriate
  • Support continuity of care and patient engagement across services

Training, Quality & Compliance

  • Lead onboarding, training, and ongoing clinical education for clinic-based staff
  • Monitor and ensure compliance with clinical protocols, standards of care, and safety regulations
  • Audit clinical and ECM documentation for accuracy, completeness, and compliance
  • Assist with performance evaluations, coaching, and staff development plans
  • Identify areas for clinical and operational improvement and support quality improvement initiatives

Administrative & Communication

  • Serve as a liaison between clinic staff and leadership
  • Assist with scheduling and staffing to ensure adequate clinical coverage
  • Participate in the development, implementation, and maintenance of clinical policies and procedures

Requirements

  • Active and unrestricted Registered Nurse (RN) license in the state of California
  • Associate’s Degree in Nursing required; BSN strongly preferred
  • Minimum 3+ years of nursing experience, with at least 1 year in a leadership, supervisory, care management role
  • Demonstrated experience or strong working knowledge of case management or Enhanced Care Management (ECM) preferred
  • Experience supervising or mentoring LVNs, Community Health Workers, and/or care coordinators preferred
  • Strong knowledge of clinical workflows, compliance standards, population health, and patient safety
  • Excellent communication, leadership, training, and conflict-resolution skills
  • Must be highly proficient with technology and able to efficiently use multiple electronic systems simultaneously in a fast-paced clinical environment.
  • Highly organized, detailed, flexible, and able to manage competing priorities in a fast-paced environment
  • Valid CPR certification
