Meadow Falls of Saybrook Assisted Living Facility

Position Title

Certified Medication Technician (CMT)

Department: Nursing

Reports To: Director of Nursing, Assistant Director of Nursing

Status: Part-time/PRN

POSITION SUMMARY

The Certified Medication Technician (CMT) at Meadow Falls of Saybrook is responsible for safely administering medications and designated treatments to residents in accordance with physician orders, facility policies, applicable state regulations, and delegated nursing responsibilities. The CMT works under the supervision of a licensed nurse and assists with resident care while promoting resident dignity, independence, and quality of life.

ESSENTIAL JOB FUNCTIONS

The CMT shall:

Medication Administration

  • Administer medications as delegated by a licensed nurse.

  • Verify resident identity prior to medication administration.

  • Follow the Five Rights (Right Resident, Medication, Dose, Route, and Time).

  • Observe residents for medication effectiveness and adverse reactions.

  • Report medication errors or concerns immediately.

  • Document medication administration accurately on the Medication Administration Record (MAR).

Medication Storage

  • Store medications according to manufacturer recommendations.

  • Secure controlled substances according to facility policy.

  • Date insulin, eye drops, inhalers, and other medications upon opening when required.

  • Maintain medication carts in a clean and organized condition.

Treatments

  • Perform delegated treatments within certification and facility policy.

  • Document treatments accurately.

  • Report abnormal findings promptly to the supervising nurse.

Resident Care

  • Assist residents with Activities of Daily Living (ADLs) as assigned.

  • Observe and report changes in resident condition.

  • Respond promptly to resident call lights.

  • Promote resident dignity, privacy, and independence.

Documentation

  • Complete documentation before the end of each shift.

  • Document medication refusals and notify the licensed nurse.

  • Record PRN medication effectiveness according to policy.

  • Maintain confidentiality of resident information.

SAFETY RESPONSIBILITIES

The CMT is responsible for:

  • Following infection prevention and control procedures.

  • Performing proper hand hygiene.

  • Using Personal Protective Equipment (PPE) appropriately.

  • Reporting accidents, incidents, and hazards immediately.

  • Following emergency procedures.

  • Maintaining a safe medication administration environment.

OXYGEN & TREATMENT RESPONSIBILITIES

The CMT will:

  • Verify physician orders before administering oxygen-related treatments within delegated responsibilities.

  • Observe oxygen safety precautions.

  • Report tubing that requires replacement.

  • Monitor residents receiving oxygen and report concerns.

  • Ensure oxygen warning signage is present when required.

SPECIAL DIETS

The CMT will:

  • Verify diet orders when administering medications with food.

  • Report discrepancies in resident diet orders.

  • Understand mechanically altered diets and aspiration precautions.

  • Communicate concerns to nursing staff immediately.

RESIDENT RIGHTS

The CMT shall:

  • Maintain resident confidentiality.

  • Respect resident choices.

  • Protect residents from abuse, neglect, exploitation, and misappropriation.

  • Report suspected abuse immediately.

QUALIFICATIONSEducation

  • High school diploma or GED preferred.

Certification

  • Ohio approved Certified Medication Technician certification

  • Current CPR and First Aide certification (or obtain within facility timeframe).

Experience

Preferred:

  • Assisted Living

  • Long-Term Care

  • Skilled Nursing

  • Memory Care

  • Confidentiality (HIPAA)

PHYSICAL REQUIREMENTS

Must be able to:

  • Lift up to 50 pounds with assistance when necessary.

  • Stand and walk for extended periods.

  • Push medication carts.

  • Bend, stoop, kneel, and reach.

  • Respond quickly during emergencies.

WORK ENVIRONMENT

The employee may be exposed to:

  • Blood and body fluids

  • Infectious diseases

  • Cleaning chemicals

  • Latex (if applicable)

  • Variable temperatures

  • Emergency situations

The CMT is expected to:

  • Demonstrate professionalism.

  • Maintain accurate documentation.

  • Complete annual competency evaluations.
