TeleMate Health QHP Provider (MD, DO, NP, PA) - AL, GA, MS, or TN licensure required

We are seeking a compassionate and skilled Qualified Healthcare Professional to join our dynamic team!
Position Overview:
As a TeleMate Health Provider, you will play a vital role in delivering patient-centered care. You will review diagnoses and medications and discuss a care plan to include daily remote monitoring and chronic care management. This position is a hybrid role, with primarily phone visits and some at-home visits, local to the provider. This position offers flexibility, work-life balance, and the opportunity to make a meaningful impact on patients lives.
Key Responsibilities:
- Conduct patient assessments, talking through health histories and diagnoses.
- Determine the necessary medical equipment per the patient's specific healthcare needs.
- Develop and implement individualized treatment plans based on patient needs and clinical guidelines.
- Provide education on the benefits of our services and be able to answer questions about the program.
- Maintain accurate and timely documentation of patient interactions in our electronic medical record (EMR) system.
Qualifications:
- Graduated from an accredited program.
- Current licensure and accreditation.
- For NP's: Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) preferred.
- Previous experience in telehealth or virtual care settings is a plus.
- Strong communication skills and a commitment to providing compassionate care.
- Ability to work independently and manage time effectively in a remote environment.
- Proficiency with electronic health record (EHR) systems and telehealth technologies.
- High speed internet at home
- Private and quiet workspace at home where phone conversations are uninterrupted and PHI cannot be overheard.
- Strong desire to provide patient advocacy
- Strongly prefer 40 hours weekly between 8AM and 3PM CST
