FIELD RN CASE MANAGER (HOSPICE) | DETAILS AND COMPENSATION:
- Location: North Forest Lake MN 55025 – 100% On-site

- Counties Covered: Washington County/Chisago County/Anoka County
- Payrate: $56.14/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
FIELD RN CASE MANAGER (HOSPICE) | SUMMARY AND HIGHLIGHTS:
The Field RN Case Manager will support hospice patients through comprehensive assessments, care coordination, and advocacy. This role focuses on delivering patient-centered, cost-effective care while collaborating closely with interdisciplinary teams to ensure optimal outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
FIELD RN CASE MANAGER (HOSPICE) | RESPONSIBILITIES:
- Perform comprehensive clinical assessments of hospice patients
- Develop and implement individualized care plans in collaboration with providers
- Continuously assess and adjust levels of care to ensure appropriateness and efficiency
- Coordinate care transitions with physicians, social workers, and discharge planners
- Arrange non-medical support services (e.g., housing, transportation) to promote treatment compliance
- Maintain detailed documentation of clinical, functional, and outcome data
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate while maintaining privacy and confidentiality
- Proactively prevent adverse patient events and intervene when necessary
FIELD RN CASE MANAGER (HOSPICE) | REQUIREMENTS:
- Education: Associate’s or Bachelor’s degree in Nursing (or related field)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active, unrestricted Minnesota RN license
- Hospice and/or home care experience
- Strong clinical judgment and ability to analyze complex medical information
- Excellent communication, documentation, and care coordination skills
