Job DescriptionPosition Summary

The Referral Specialist is responsible for coordinating referrals for patients at assigned clinic(s). This includes submitting, tracking, following up, and closing referrals, while obtaining necessary verification and authorizations as directed by the Primary Care Provider. The Referral Specialist plays a vital role in ensuring continuity of care and supporting our mission to deliver exceptional patient-centered healthcare.

Key Responsibilities

  • Submit, track, and close all referrals efficiently and accurately.

  • Obtain and log authorizations for specialist referrals and/or procedures.

  • Gather required documentation (clinical notes, labs, imaging, etc.).

  • Enter referral information into the electronic health record (EHR).

  • Fax referral information and clinical documents to provider offices before appointments.

  • Monitor pending referrals, update status, and send letters to patients per policy.

  • Communicate with provider offices to obtain referral notes and scan/index received records.

  • Close expired referrals and obtain retroactive authorizations as needed.

  • Review pending authorizations daily and coordinate with insurance payers to ensure timely care.

  • Keep care team members informed of issues affecting referral completion.

  • Provide coverage for other clinic team members or CHNWF locations as needed.

  • Perform other duties as assigned to support clinic operations.

Minimum Qualifications

  • High school diploma or equivalent.

  • Knowledge of medical terminology and experience in a medical office setting.

  • Intermediate computer skills (Microsoft Office Suite and EHR systems).

  • Ability to complete all assigned patient EMR functions accurately.

Preferred Qualifications

  • Accredited Medical Assistant training.

  • Previous success in customer service.

  • Experience working in a health clinic and demonstrated commitment to serving low-income populations.
