Job DescriptionJob Title: Licensed Practical Nurse (LPN) – LTC / OPWDD

Location: Wayland, NY
Duration: 26 Weeks
Schedule: Night Shift – 4x10 Hours (Sunday–Wednesday)
Shift Hours: 10:00 PM – 8:00 AM
Hours: 40 Hours Per Week
Employment Type: Contract

Compensation

Pay Rate: $32 - $38/Hour (Based on Experience)

Position Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a Long-Term Care and Developmental Disabilities setting. The LPN will work under the supervision of a Registered Nurse and physician to support individuals with developmental disabilities by delivering direct patient care, administering medications, monitoring health conditions, and promoting a safe and supportive environment.

ResponsibilitiesPatient Care & Clinical Support

  • Administer medications, treatments, and injections as prescribed.
  • Perform tube feedings and other basic clinical procedures within scope of practice.
  • Monitor, record, and report vital signs including temperature, pulse, respirations, and blood pressure.
  • Observe and promptly report changes in patient condition to supervising nursing staff and healthcare providers.
  • Provide wound care, dressing changes, and colostomy care as directed.
  • Assist with activities of daily living including bathing, grooming, dressing, feeding, and mobility assistance.
  • Support patient comfort, dignity, and independence while maintaining quality care standards.

Documentation & Compliance

  • Maintain accurate and timely patient documentation in electronic medical record systems.
  • Follow established care plans and document patient progress and responses to treatment.
  • Adhere to infection prevention, safety, and regulatory guidelines.
  • Ensure compliance with all facility policies and standards of care.

Laboratory & Clinical Procedures

  • Collect specimens as ordered.
  • Perform venipuncture and blood collection if appropriately trained and certified.
  • Assist with monitoring and managing ongoing patient health needs.

Team Collaboration

  • Collaborate with Registered Nurses, physicians, and interdisciplinary care teams.
  • Participate in care planning and communication regarding patient status and treatment goals.
  • Float to other units or assignments as needed based on operational requirements.

Environmental & Resident Support

  • Assist in maintaining a clean, safe, and organized patient environment.
  • Support management of patient belongings, linens, and personal care items.
  • Promote patient safety and well-being at all times.

Required QualificationsEducation

  • High School Diploma or equivalent required.

Licensure & Certifications

  • Active New York State Licensed Practical Nurse (LPN) License required.
  • BLS/CPR Certification preferred or required per facility policy.
  • Venipuncture Certification preferred (if applicable).

Experience

  • Previous Long-Term Care (LTC) experience preferred.
  • Experience working with individuals with developmental disabilities preferred.
  • Strong understanding of patient-centered care and nursing best practices.
